Please fill out the Health Questionnaire and bring with you to your first appointment. This form provides us with necessary health information that will assist us in giving you the best care possible.  You may complete this form in the office if you prefer; however, please arrive to your appointment at least 20 minutes early to ensure you have sufficient time to complete the form.

Your First Visit

Here are some guidelines to get the most out of treatment and know what to expect.

At your first appointment we will spend time discussing your health history and your chief complaint(s). There will be a physical examination, including tongue and pulse diagnosis.  Then needles are inserted and retained for 30-45 minutes depending on the treatment. The initial appointment is usually lasts up to 90 minutes, and follow up visits are 45 minutes to 1 hour.

We recommend you have a small meal a few hours prior to treatment.  It is not recommended that you receive treatment if you are very hungry or very full.  Please refrain from strenuous exercise and alcohol before and for several hours after treatment. 

It is important to drink plenty of water before and after any kind of bodywork, including acupuncture.  Your entire body works best when you are fully hydrated. Staying hydrated will help help your body function better and assimilate the changes that an acupuncture treatment actuates.

We recommend you wear loose fitting clothing for treatments so that you are comfortable and treatment sites are easily accessible. You are welcome to bring a change of clothing to wear during treatment if you prefer.

Treatment Frequency and Duration

The number of treatments required varies between individuals depending upon the health of the patient, the severity of the condition and the length of time the condition has been present. Usually, the longer a condition has been present the longer it will take to resolve.  On your first visit we assess treatment factors and set realistic goals. It is generally recommended that patients begin with treatments once or twice a week to build momentum and as the condition improves frequency is reduced. Often patients get so much value from acupuncture that they continue with monthly or even weekly maintenance treatments for wellness and preventative care.

Insurance Verification

Our practice accepts insurance for acupuncture patients. However, your coverage may vary depending on your specific plan and your diagnosis. In order to best serve you we can verify your insurance benefits before you arrive. Please click on the link below and complete the online form. We will contact you as soon as the benefits are established.

If you do not have insurance coverage or our services are not covered under your plan, then you will be informed of alternate payment options and time of service fees for acupuncture treatments..

All services are by appointment only.

Appointments can be made by phone. [or on-line scheduling]

Please be on time for your appointment. If you find you are running late, please call and let us know. Accommodations will be made to the extent possible; however, this cannot be guaranteed.  If an appointment must be shortened due to lateness, normal treatment fees may still apply.

Payments

Payment by cash, check, debit or credit is due in full at time of service.

Cancellations

In fairness to both new and returning patients our office maintains a 24 hour cancellation policy.  New patients may be required to provide a credit card number (including 3 digit CVV code) when scheduling new patient visits.  When booking appointments, patients agree to pay the entire fee for a visit if they fail to provide 24 hour prior notice of cancellation.
